PRC Licensure Examination Application Requirements & Procedures

BASIC REQUIREMENTS:
1.) Original & photocopy of transcript of records w/ complete date of graduation, S.O. number, scanned picture & w/ remarks: “for board examination purposes only”.
2.) Original & photocopy of birth certificate (& marriage certificate for married females) issued by the NSO.
3.) Current community tax certificate.
4.) Three (3) copies recent passport size colored picture, white background w/ complete name tag.
5.) Examination fee of nine hundred pesos (P900.00) for baccalaureate degree and six hundred pesos (P600.00) for non-baccalaureate degree.
6.) Original and photocopy of birth certificate and marriage contract from the Local Civil Registrar (LCR) if NSO copy is not legible

ADDITIONAL REQUIREMENTS:

FOR ARCHITECTS:
1) Latest NBI clearance
2) PTR IAPOA number of mentor
3) Logbook of diversified experience
4) Affidavit of Experience signed by a registered Architect with updated ID

FOR AGRICULTURISTS, LIBRARIANS and FISHERY TECHNOLOGISTS:
1) Certification of Good Moral character (Barangay, Parish Priest or Employer/School)

FOR CHEMISTS:
1) For BS in Chemistry – thesis should be indicated in the Original Transcript of Records (OTR)

FOR CRIMINOLOGISTS:
1) Latest NBI clearance or Police clearance and Court clearance
2) Certificate of Good Moral Character from at least three (3) persons of good standing (Barangay, Parish Priest and Employer/School)

FOR ENVIRONMENTAL PLANNERS:
1) Certification of Good Moral character
· Masteral degree in environmental planning or its equivalent acceptable to the Board
· Bachelor’s degree in environmental planning or its equivalent and with two (2) years of environmental planning experience certified by a registered environmental planner
· Masteral degree in architecture, civil eng’g., economics, public administration, or sociology and with one (1) year of environmental planning experience certified by a registered environmental planner
· Bachelor’s degree in either architecture, civil eng’g., economics, public administration, or sociology and with two (2) years of environmental planning experience certified by a registered environmental planner

FOR ELECTRONICS TECHNICIANS:
1) Must be a graduate of a two (2) year Associate, Technician, trade or Vocational course in electronics or has completed at least 3rd year of BS ECE

FOR FORESTERS:
1) Two (2) certifications of Good Moral Character (Barangay, Parish Priest or Employer/School)

FOR INTERIOR DESIGNERS:
1) Must be any of the following:
2) BS in Interior Design or mater of Interior Design
3) BS in architecture
4) Bachelor of Fine Arts major in Interior Design
5) Any Baccalaureate degree with 60 units in Interior Design

FOR GUIDANCE COUNSELORS:
1) Original and photocopies of Original Transcript of Records and Diploma in Baccalaureate and Masteral degrees
2) Latest NBI clearance

FOR LANDSCAPE ARCHITECTS:
1) Must be any of the following:
Ø BS or post graduate degree in Landscape Architecture
Ø BS Architecture with 20 units of Landscape Architecture design courses or with at least five (50 years experience in the field of Landscape Architecture
Ø Bachelor’s degree in Horticulture with at least forty (40) units of Landscape Architecture or with at least five (5) years experience in the field of Landscape Architecture

FOR NURSES:
1) Summary of Related Learning Experience (RLE)
2) Records of O.R., D.R. and Initial Cord Care Cases
3) Certificate of Authenticity to accompany the application for NLE ( available at PRC Application Unit)

FOR MASTER PLUMBERS:
1) Non – degree holders / High School:
a. Notarized Certificate of Experience for five (5) years signed by a Reg. Master Plumber whose date of registration is prior to the date of employment
b. Certificate of Employment or service record
2) For baccalaureate degree holders:
a. Graduates only of Architecture, Mechanical Engg., Civil Engg., Elect. Engg.

FOR CERTIFIED PLANT MECHANICS:
1) At least one (1) year certificate of experience signed by a Reg. Mechanical Engineer or Professional Mechanical Engineer with updated ID
2) Certificate of Employment or service record
3) Detailed description of equipment
4) Affidavit of competency

FOR MIDWIVES:
1) For Graduates of Midwifery:
a. Record of twenty (20) deliveries actually handled, signed by Principal or Dean and duly notarized
b. Record of five (5) sutures and five (5) intravenous injections
2) For registered Nurses:
a. PRC ID card / Board Certificate
b. Record of twenty (20) deliveries actually handled with record of five (5) sutures and five (5) intravenous injections signed by Hospital Director or Municipal Health Officer and duly notarized

FOR NUTRITIONIST-DIETICIAN:
1) For graduates of BS in Food and Nutrition only
a. Certificate of completion of practical training in:
Ø Public Health – 180 hours
Ø Adm. Dietetics – 160 hours
Ø Clinical Dietetics – 192 hours

FOR PHARMACISTS:
1) Daily Time Record (DTR) and Certificate of Internship signed by a registered pharmacist with updated ID in:
a. Manufacturing/Laboratory – 160 hours
b. Community – 160 hours
c. Hospital – 160 hours
d. Major Internship in any of the above – 480 hours

FOR PHYSICAL THERAPISTS and OCCUPATIONAL THERAPISTS:
1) Certification of nine (9) months of internship signed by the dean and registrar with school seal

FOR PHYSICIANS:
1) Certificate of Post Graduate Internship

FOR REG. MASTER ELECTRICIANS:
1) Notarized Certificate of Experience signed by an REE or RME whose registration is prior to the date of employment
2) Certificate of Employment or service record
Ø For BSEE graduates / completed three (3) years BSEE or three (3) year course in Electrical Technology – has completed one (1) year practice in electrical wiring and installation, operation and maintenance
Ø Two (2) year vocational electrician’s course – has completed two (2) years apprenticeship in electrical installation, operation and maintenance
Ø One (1) year vocational electrician’s course – has completed three (3) years apprenticeship in electrical installation, operation and maintenance
Ø High School graduates – has completed five (5) years apprenticeship in electrical wiring installation, operation and maintenance

FOR SOCIAL WORKERS:
1) Certificate of 1,000 case hours signed by a registered Social Worker with updated ID

FOR ACCOUNTANCY, CHEMICAL ENGINEERS, DENTISTS and VETERINARIANS:
1) Latest NBI clearance

PROCEDURES

1. Download the Licensure Examination Application Form (Except NLE and LET).
2. Print in a single legal (8.5″ x 14″) sized bond paper (back-to-back).
3. Accomplish the form using a BLACK INK ballpoint pen.

5. Bring the accomplished form to PRC Baguio Regional Office along with the requirements listed above for further assessment and verification.
